1. Dental hard tissue pathologies, aetiology, and their clinical manifestations
damage or defect of tooth – caries , toothwear , trauma , developmental condition
principle – Detection , Dx , control/prevention
4 conditions – ให้ดู – etiology , histopatholgy , microbiology
1.2 Dental caries
ถือว่าเป็น reversible ใน earliest stage , กระตุ้นโดย bacteria , fermentable carbohydrate ใน plaque biofilm -> acid demineralization -> proteolytic destruction of organic component of dental tissue
1.2.2 Terminology
Primary caries – เกิดใน tooth surface
Root caries – เป็น 1* caries ที่ expose มาถึง root surfce , มักเกิดใน gingival recess , penetrating ง่ายกว่า
pathology of primary and root caries เหมือนกัน
Recurrent(2*) caries – เป็น primary caries เกิดที่ margin of restoration

1.2.3 Caries process and lesion
The carious process – metabolic activity ในplaque biofilm , biofilm เริ่มไม่กี่ นาทีหลังแปลงฟัน
absorb คล้าย acquired pellicle เยื่อบางๆ ใน salivary prot and glycoprotein – oral bac colonized with bacterial polysaccharide and salivary protein
– ผ่านไป -> increase density of developing biofilm – changing bac population , ph , oxygen tension และสร้าง cariogenic environment ที่ tooth surface
การ de- and remineralization metabolic process – สามารถ modified โดย regular biofilm ,toothbrush , fluoride toothpaste
ถ้า biofilm ถูก partial or totally remove -> การเกิด mineral loss จะถูก stop or reverse , mineral gain (ใน very early lesion)
fluoride ใน toothpaste จะdelay lesion progression – โดยการ inh demineralization , และ encouraging remineralization
