Keloid Treatment – Overview in English

1. General Principles
Keloids are overgrowths of scar tissue that extend beyond the original wound boundaries due to excessive collagen production. Treatment aims to flatten the scar, reduce symptoms (itching, pain), and prevent recurrence. Because recurrence rates are high, a combination of therapies is often recommended.

2. Common Treatment Options

  • Corticosteroid Injections
    • Triamcinolone acetonide is commonly injected into the lesion every 4–6 weeks.
    • Reduces inflammation, softens the scar, and can flatten keloids over time.
  • Silicone Gel Sheets or Ointments
    • Applied over the keloid for several hours daily.
    • Maintains hydration and reduces scar thickness.
  • Pressure Therapy
    • Compression garments or devices apply constant pressure to the scar.
    • Often used after burns or ear piercing keloids.
  • Cryotherapy
    • Liquid nitrogen is used to freeze the keloid.
    • Works best for small, early lesions; may be combined with steroid injections.
  • Laser Therapy
    • Pulsed dye laser or fractional CO₂ laser reduces redness and thickness.
    • Usually combined with other treatments for better results.
  • Surgical Excision
    • Physical removal of the keloid.
    • High recurrence risk unless combined with adjunctive therapies such as immediate post-op steroid injection, radiation therapy, or silicone sheeting.
  • Radiation Therapy
    • Low-dose radiation given after surgical removal to prevent recurrence.
    • Requires careful risk–benefit assessment.

3. Supportive and Preventive Measures

  • Avoid unnecessary skin trauma (tattoos, piercings).
  • Begin preventive treatment early in high-risk individuals after surgery or injury.
  • Monitor closely for early signs of recurrence.
