มีการ overuse MRI , หรือ อาจจะเกิดกรณี dismiss จากผล MRI ยังได้ เพราะอาจจะมี abnormal จาก MRI ที่พบได้ในคนไข้ทั่วไป
เมื่อเจอสิ่งผิดปกติจาก MRI ก็อาจจะต้อง Diagnostic testing เช่น
– Epidural steroid injection
– Lumbar decompression or fusion
มี CPGs – Clinical practice guidlines จาก evidence bases หลายๆร้อย study
– แต่อย่างไรก็ตาม CPGs ก็อาจจะไม่สามารถแทน expert clinical judgement ได้หมด
– แบ่งเป็น acute (<12 สัปดาห์) และ chronic (> 12 สัปดาห์)
– จาก CBRG (Cochrane Back Review Group)
– บางเคสถูกตัดจาก study เช่นมี neurologic involement , และยังตัดอีก 5 ข้อ เช่น serious spinal pathology , มี specific cause of LBP , มีneurologic involvement , มี severity of symptom และ functional limitation , และสุดท้ายเป็น risk factor ต่อ chronicity
