p 390 ACD – allergic contact dermatitis เป็น cell-mediated (Type IV) ,delay type k, hypersensitivity reaction – เกิดจาก skin contact กับ env allergen
ต้องเริ่มจาก Prior sensitization -> ก่อน allergy develop
ACD – เป็น aczematous dermatitis – ac phase มีss pruritis , erythema , edema , vesicle
– confine area direct exposure
– recurrent contact จาก causative allergen -> เกิด ch disease , characterized โดย lichenified erythematous plaque, c variable hyperkeratoiss , fissuring , pigmentary change อาจจะ spread beyond area direct exposure
itch ,swelling -เป็น key components
patch testing Dx test -เป็น choice ในการ identify causal allergen
– ทำในผปที่มี persistent/recurrent dermatitis และ suspect ACD
Rx หลีกเลี่ยง allergen
ACD พบ 20% new case of contact dermatitis
– ที่เหลือเป็น irritant contact dermatitis
ACD อาจจะเรียกว่า adverse cutaneous inflammatory reaction – จาก contact specific exogenous allergen , จาก previous sensitized
