pigmentation ที่เกิดจาก UV exposure
UVA – induce 3 type of pigmentation
1. IPD – immediate pigment darkening – เกิดเร็วเป็น minute , และ fade เร็ว
2. PPD -persistent pigment darkening – develop ในหลายๆชม , และอาจจะ persist นานเป็น wk , ขึ้นกับ UVA dose and skin type
3. Delayed pigmentation start หลังจาก some day ,long lasting <- เกิดจาก increase melanin synthesis
UVB – induced สี tanning เป็น delayed pigmentation จาก melanin synthesis และมักเกิดจาก burn , และหายไปเมื่อ epidermal turnover
ใน darker skin – UVA จะมี pigmeting effects มากกว่า UVB

common- childbearing age , dark-skin asian , african
women : men 1.5-33.3% , men 10%

freckles , solar lentigo , postinflammatory hyperpigmentation , nevus of Ota , nevus of Hori , pigmented contact dermatitis , Lichen planus pigmentosus , frictional melanosis , facial acanthosis nigricans , Becker’s melanosis
Hx – chemical exposure
Wood’s lamp , dermoscopy , concomitant inflammatory disorder
Treatment
cosmetic camouflage
over-correction and feminization ควรหลีกเลี่ยง
men ชอบ solution มากกว่า cream or ointment
stinging sensation จาก high ethenol ตอน apply shaving อาจจะทำให้เกิดปัญหาได้
ไม่ต้อง fragrance มาก
sun protective , use sunscreen, hat , clothign
Rx – depigmenting cream
oral depigmenting agent
chemical peel
microdermabrasion
dermabrasion
mesotherapy
light and laser
platelet rich plasma therapy
บางเคสอาจจะ response monoRx หรือบางคน combination
Broad-spectrum UVA,UVB + physical block eg zinc oxide or titanium dioxide c minimum sun protection factor SPF 30
-ช่วยป้องกัน melanocyte reactivation จาก sun exposure
braod-spectrum ช่วยเพิ่ม efficacy of hydroquinone
Topical depigmenting อาจจะ alone or combination – hydroquinone , topical corticosteroid , retinoic acid , azelaic acid , kojic acid , arbutin , licorice extract , ascobic acid and soy
study – combine 2% mequinol + 0.01% retinoic acid …12 wk
topical rx with fixed triple combination เป็น 1st line
MonoRx or dual Rx – lower efficacy , slower onset <- ใช้กรณีที่ 1st line ใช้ไม่ได้
