在上下睑缘距外眦 5–10 mm 处，用 4-0 Vicryl 缝线做牵拉缝。
局部麻醉为 0.5 mL 混合液，成分为 2% 利多卡因加肾上腺素（1:1,000），
注射到设计区域。
使用 11 号 Bard-Parker 手术刀沿灰线由内向外切开，
然后用 Metzenbaum 剪刀将切口向内延伸约 3 mm，
以便分离睑结膜。
之后用钝性十切剪进行剥离，
在上外侧和下外侧方向充分分离，
以确保反向 V-Y 皮瓣完全游离。
仔细止血并用电凝器（Bovie electrocoagulator）
对眼轮匝肌进行分离。
当分离完成时，
虽然睑结膜切口仅有 3 mm，
但由于结膜和灰线的粘弹性特性不同，
皮瓣仍可推进 5 mm ——
特别是灰线的弹性比结膜小。
随后在 5 个点位放置锚定缝，
既用于固定，也用于牵拉，
采用 5-0 黑色丝线缝合。
缝线通过 Steri-Strips 与邻近皮肤固定，
以折叠方式加固，
每根缝线均用 3 层互锁的 Steri-Strip 胶带相互叠加，
并向相反方向折叠固定。
牵拉缝保留 10 至 14 天，
期间仅在随访清洁切口时更换 Steri-Strips
（如图 2 所示）。
