pain – unpleasant sensory and emotional experience asso potential or actual tiss damage
ac pain – < 3 mo
ch pain – > 3-6 mo
qualify pain – บอกยาก – subjective , ใช้ 10 point scale
ch pain แบ่งเป็น 3 types
– Nociceptive – direct tiss damage
– Neuropathic – direct nerve
– central – damage cns ~ intracranial pathology
ทำให้ hypersensitization
ทั้ง ac and ch pain – ตกอยู่ใน broad category ข้างต้น
orofacial or neck pain
orofacial pain – มี broad type of pain เพราะมีmany different neural inputs and outputs
แบ่ง orofacial pain 2 groups
– Peripheral pain – tooth abscess , tmj , dental caries
– central pain – intracranial mass ไปกด trigeminal n , vascular insuf -ไป Modulate input/output pain signal
แยก ระหว่าง Head / neck pain , เพราะมันจาก stem ต่างกัน
eg migrain บางที refer to oral cavity หรือ facial regions
mc pain ที่ relate tmj
ถ้ามีmood disorder – ทำให้ worse pain
ตอ้ง rx ทั้ง mental and emotional
พยายามไม่ Missing something bigger , eg infection
– จากนั้นค่อย address psychological factors
rx anti-depressant , anti-anxiety or cognetive behavior rx
เพื่อ reset mind ให้ resetting perception of pain cycle
Factor affecting perception of pain eg
– gender ,women more perception pain มากว่า Men ดฉ ch pain
– orofacial pain – พบไม่มาก 10-26% incident , women more affected orofacial pain
2nd pain eg osteoarthritis , spine
การ rx ch pain บางทีดีขึ้นจาก placebo – psychological
app pain mx – multimodal fashion
– pain mx -> removing cause
-> medications -> anti-inflam , neuropathic , anti-depress , opiods
– opiod ถ้าใช้ ต้อง short course, หรือ just trial
และให้กรณี during sx , after sx ,
ส่วน ch pain – มี evidence ไม่ได้ผลเท่าไร
mo < 50mg /day ideal , ถ้าเยอะกว่านี้ ก็ระวัง overdose , apnea
สำหรับ orofacial pain – มักจะแค่ minimized opioid , eg after tmj sx
– ใช้ nsaid – ibuprofen , ketorolac(แรงกว่า ใช้ periop) , meloxicam หรือ tylenol แทน
ถ้าเกี่ยวกับ n จะใช้ anti-convulsants
– gabapentin
– pregabalin
– carbamazepine
– oxcarbazepine
– topiramate
