p 1 pneumonia – involve – complex step , เริ่มจาก contact pathogenic microorganism , culminating invasion of lower resp tract
มาได้ทั้งใน และนอก รพ
transmitted by aspirate , inhale microorganism
ได้ทั้ง bac , viral , fungal , แต่ mc มักเป็น bac
Overview
hospital-acquired or nosocomial pneumonia HAP – develop 48hr or after adsmission
ไม่ได้ incubating at time of admission
p 2 เกิดตอน intubated ก็ได้
HAP เป็น 2nd mc nosocomial infection 0.5-1.7% hospitalized pt
– เป็น dynamic disease , change natural history , mutiple etio , numerous risk factor
ventilator-asso pneumonia (VAP) พบได้ 48-72 hr หลังใส่ ET , และได้เยอะ 70-80% of HAP
pneumonia พบได้ถึง 1/4 ใน ICU , และ 2/3 เป็นแบบ nosocomial
HAP แบ่งเป็น 2 group
1) early onset
2) late onset
