🔍 Clinical Assessment

  • Residual Ptosis: The right eyelid still appears more droopy than the left.
  • Frontalis Recruitment: Current frontalis action elevates but does not sufficiently open the fissure.
  • Symmetry Issue: The aperture is asymmetric, likely due to under-correction or loosening of the suspension.

✂️ Surgical Options for Retightening

  1. Frontalis Sling Tightening / Adjustment
    • Re-open the previous incision and shorten or adjust the sling material (fascia lata / silicone / ePTFE).
    • This increases lift and reduces lag.
  2. Reinforcement with Secondary Sling
    • If the current material has stretched, a new sling (silicone rod or fascia lata) can be added for more durable lift.
  3. Combination with Levator Resection (if residual function)
    • If levator has even mild function (≥3 mm), combining sling tightening with levator advancement can give a more natural lid contour and better aperture.

⚠️ Risks and Considerations

  • Overcorrection → lagophthalmos, corneal exposure.
  • Asymmetry → common after tightening, so intraoperative adjustment in sitting position is essential.
  • Material issues → silicone can loosen; fascia lata more stable but invasive.

✅ Recommendation

  • Perform tightening of the frontalis suspension (check material tension, shorten sling).
  • Intraoperative adjustment with patient sitting to balance both eyes.
  • If sling material is stretched or fibrosed → consider redo with autologous fascia lata for stability.
  • Post-op lubrication is mandatory to avoid keratopathy.

actually , this case come to me after 1yr operation, need less crease height, so i adviced more tightening for decrease crease too , and didn’t make crease after tightening

🔎 Case Understanding

  • Patient presented 1 year after frontalis sling surgery.
  • Main concern: high eyelid crease.
  • Your approach: further tightening of the sling to both improve eyelid opening and reduce crease height.
  • Important: no new crease fixation sutures were made after tightening.

🧩 Rationale

  • Sling tightening reduces crease height:
By increasing the frontalis pull, the tension between skin and tarsus is redistributed. This lessens the definition of the previous high crease, making it appear lower and more natural.
  • No additional crease sutures:
Prevents excessive adhesion/scarring between skin and tarsus, avoiding an unnaturally deep or stiff fold.

🛠 Surgical Steps (Summary)

  1. Incision: Re-enter through the previous incision.
  2. Dissection: Carefully separate subcutaneous tissue and identify sling material, preserving existing adhesions.
  3. Tightening: Shorten or reinforce the sling to increase eyelid aperture.
  4. No crease sutures: Do not suture skin to tarsus; allow natural redistribution of tension to lower the crease height.
  5. Adjustment: Intraoperative sitting position check for aperture and crease level, then close skin.

⚠️ Key Considerations

  • Crease improvement may be limited if scarring is dense or skin is thin.
  • Avoid over-tightening to prevent lagophthalmos or exposure keratopathy.
  • Skin thickness factor: Thicker skin patients show better reduction in crease height, while thin-skin patients require more caution.

Principale plainte : pli palpébral trop haut.

Principale – main, primary
plainte – complaint, chief concern
pli – fold, crease
palpébral – palpebral (relating to the eyelid)
trop – too, excessively
haut – high

Queja principal: pliegue palpebral demasiado alto.

⚠️ Puntos clave a considerar
