– และไม่ transparent
– inner layer of sclera (lamina fusca) – blend กับ uveal tract
– Anterior episclera ประกอบด้วย connective tissue ระหว่าง superficial sclera stroma และ tenon capsule
– มี 3 pre-equatorial vascular layer

2. Superficial episcleral plexus

orbital fascia แบ่งเป็น
– fascia bulbi -> หรือ Tenon’s capsule
Tenon’s capsule คลุมตั้งแต่ด้านหน้า เว้น corneal , ไปด้าน optic nerve , และด้านหน้ายังไป merge กับ conjunctiva
Optic nerve แล้วเป็น anterior and posterior , ตรง posterior เป็นส่วนที่ tenon’s ไป merge กับ dura sheath
– muscular sheath
– intermuscular septum
– EOM expansion
– Ligament of Lockwood : ด้านล่างของ eyeball , Tenon’s ไปรวมกับทำให้เป็นจุดยึดแข็งๆเพื่อความ stable of eyeball
Synergist effect of superior rectus muscle and LPS muscle , ทำให้ตอนแก้ ptosis ถ้า LPS resection ไม่ดี อาจจะทำให้เกิด hypotropia
– และ inferior rectus muscle ก็ attach กับ capsular fascia

systemic -> collagen vascular disease eg rheumatoid arthritis , herpes zoster ophthalmicus , gout
พบบ่อยถึง 1/3 pt ใน center
infectious – very rare
inv ในกรณี recur -> scleritis
Simple episcleritis – 75% of cases
มักจะ recur 60% , แต่เมื่อผ่านไปเรื่อยๆ ความถี่จะลดลง
peak 24hr , ค่อยๆ fadeลง few next days
ss : redness , discomfort จาก absent to 50% , moderate , occasionally severe
R/o – scleritis , อาจจะมี grittiness , photophobia
sign มากกว่า 1/2 พบ bilatr
– VA – normal
– redness – sectoral , 2/3 หรือ diffuse , และมักมี interpalpebral distribution
triangular config ที่ base at limbus
-chemosis , ocular HT , antr uveitis , keratitis – rare
